Pupil Profile 

Each child in a SU will have a Pupil Profile linked to the Outcomes on their EHCP. We use this plan to set specific targets for your child. These targets will be set and reviewed termly. They will be achievable steps of progression towards their long term outcomes, giving your child the best possible chance to succeed.

Annual Review

Every child in a SU has (or is in the process of receiving) an Educational Health Care Plan that is reviewed and updated yearly. The Annual Review will be held a year after the final plan was agreed. A letter will be sent home in advance to invite you to the meeting. During the meeting your child’s EHCP outcomes will be reviewed, progress noted and achievements celebrated. New short-term targets towards their outcomes will be set for the coming year. Once the Annual Review paperwork is completed the document will be sent to the Local Authority and they will make amendments suggested to the review and issue an amended draft.

Early Annual Review for Phase Transfer 

What is a 'phase transfer'? 

Local Authorities ('LAs') have a legal duty to review and amend an Education, Health and Care (“EHC”) Plan when a child or young person transfers from one phase of education to another. Phase transfer is the moving between particular stages of education. At Castle Hill SUs, this will take place in Year 2 and Year 6. School will support you during this process - all documents need to be completed and submitted to the LA by the 31st October. The LA has a responsibility to name the next placement school by the 15th February.
